The best way to register as a new patient is to download a registration form the bottom of the page, complete it and return it to the practice either in person or by post or by email to email@example.com

Registering as a Patient
As a fully registered patient of this practice you will be registered with a specific GP who will oversee your care to help provide continuity. However, you are free to see any of the doctors or nurses at the practice as you so wish.
Please print out and complete the appropriate registration forms (see links below) and bring them into the surgery on your first visit to the practice. You will need to complete a separate form for each member of your family.
If you do not live within the catchment area (see below) you can still register at Brixton Hill Group Practice as an 'Out of Area' patient. Regrettably we can not offer home visits to out of area patients nor can we register families with children under 5 'out of area' due to health visitor provision. Out of area patients are registered at the practice's discretion.
EMIS Online Patient Access
If you would like access to EMIS Online Patient Access please complete an Online Access Form (see link below) and bring this into the surgery along with ID such as driving licence or passport as well as separate proof of address, such as a utility bill or a tenancy agreement.
Generally the best time to come and register is between 8:30-11:00 am and 3:30-6:00 pm, as more staff will be available to assist you during these times.

CATCHMENT AREA (for provision of home visits) If you live outside of this catchment area you can still registrar as an out of area patient.
