Registering as a Patient

Please check the map below to make sure you live within the catchment area of the practice.

Please print out and complete the appropriate registration forms (see links below) and bring them into the surgery on your first visit to the practice. You will need to complete a form for EVERY member of your family.

As a fully registered patient of this practice you will be registered with a specific GP who will oversee your care to help provide continuity. However, you are free to see any of the doctors or nurses at the practice as long as an appointment is available.


EMIS Online Patient Access

If you would like access to EMIS Online Patient Access please complete an Online Access Form (see link below) and bring this into the surgery along with ID such as driving licence or passport as well as separate proof of address, such as a utility bill or a tenancy agreement. 

 Generally the best time to come and register is between 8:30-11:00 am and 3:30-6:00 pm, as more staff will be available to assist you during these times.
